    Our CNA’s are the lifeblood of our facility, providing compassionate care for residents and support for our nursing staff. Full and Part Time positions available. New Graduates Welcome!

    Responsibilities

    • Providing compassionate and quality care to our residents
    • Ensuring residents basic personal needs are met by assisting with walking, feeding and bathing
    • Assist residents in the use of walkers and similar equipment in a prescribed physical therapy program
    • Keep residents’ space clean and tidy to help them feel safe and comfortable
    • On occasion, assist with clerical tasks
    • May accompany or assist resident/residents in supportive nursing services and transporting of resident/residents in ambulance or clinic car

    Our nurses are the lifeblood of our facility, providing compassionate, expert care for our residents. We are looking for experienced Licensed Practical Nurses who will be responsible for the routine nursing care of residents fulfilling a detailed and well-defined resident care plan. Full and Part Time positions available.

    Responsibilities

    • Responsible for the delivery and coordination of quality resident care in compliance with physician order
    • Ensure the highest level of resident comfort and dignity
    • Recognize changes in resident needs and report critical changes to an RN
    • Assist in the implementation of the nursing care plan
    • Take/record vital signs and observes and documents other signs and symptoms as directed
    • Assist in carrying out diagnostic procedures under supervision
    • Responsible for clinical record documentation
    • Assist in the admission and discharge of residents
    • Assist residents in performing ambulation exercise and maintaining proper joint motion
    • Help maintain equipment used in care of residents as directed
    • Perform tasks designed to provide a comfortable environment for residents
    • Assist residents with personal hygienic care, including bathing
    • When acting as charge nurse, has responsibility for supervising over other LPN’s and CNA’s
    • May participate in Quality Assurance monitoring and Audits. May serve on Quality Assurance subcommittees

    Our staff are the lifeblood of our facility, providing a comfortable, home-like experience for our residents. We are seeking a Food Service Helper who will assist in the preparation and service of food to residents, along with the cleaning of kitchen equipment. On occasion Food Service Helpers may perform cooking duties.

    Typical Work Activities

    • Assist the Cooks in the preparation and service of meals
    • Prepares food carts and safely delivers trays to our 8 country kitchens
    • Helps serve food portions to residents
    • Prepares cold foods within established preparation guidelines
    • Reads menu portions carefully and arranges food accurately using the proper guidelines
    • Practices good food handling procedures and correct use of food gloves and serving utensils
    • Assists in proper storage of food to prevent spoilage
    • Prepare, maintain and organize dining areas, including setting tables
    • Clean kitchen, dining areas, dishes, pots and pans, linens and storage areas according to established guidelines
    • May serve as cashier

    Our staff are the lifeblood of our facility, providing compassionate, quality care for residents. We are seeking a Staff Social Worker to provide support to our residents, some with social, emotional and related difficulties associated with behavioral health disorders.

    Responsibilities

    • Participate in the intake and screening of new residents, evaluating and diagnosing the resident and their problems, and in the development of treatment plans
    • Collect information pertaining to the resident’s history including education, medical, psychological, and social experiences and supported by a psychiatric evaluation
    • Provide social work services to help residents recover from their disabilities and social environment
    • Refer residents to other governmental and non-governmental agencies to obtain services to meet the resident’s needs
    • Coordinate various activity and treatment programs with other social agencies, such as psychology, medical, psychiatric, social, educational, legal, housing, and financial services
    • Record necessary pertinent information per State rules, regulations and local policy utilizing a personal computer
    • Evaluate a resident’s response to services obtained in accordance with the treatment plan and makes modifications and changes as needed to ensure stability
    • Meet with supervisors and team members to discuss a resident’s progress in response to the treatment plan
    • Plan with the clinical team and with community agencies to relate the resident’s treatment and rehabilitation program to his social situation prior to his discharge
    • Prepare a discharge plan for less structured programs for residents who are stable and no longer require departmental services
    • Maintain information on community and health resources which can be utilized during the resident’s care and subsequent to his discharge
    • Meet with service providers to evaluate resident’s progress for services received during the resident’s care and subsequent to his discharge from the mental health program
    • When working with Mobile Crisis Team, may be designated as shift leader or assistant shift leader in overseeing the non-supervisory responsibilities of the clinical operation of an assigned shift in the absence of the supervisor

    Knowledge, Skills, Abilities and Personal Characteristics

    • Good knowledge of the practices and procedures of social work and of the social factors affecting disease and disability
    • Good knowledge of community resources available to provide services to the mentally ill
    • Ability to get along well with persons in need of behavioral health services
    • Ability to organize casework effectively
    • Ability to receive and follow oral and written direction;
    • Ability to interpret federal, state and local laws affecting the medical and health programs
    • Ability to prepare reports and maintain records utilizing a personal computer
    • Mental alertness; tact and courtesy; good judgment
    • Good powers of observation and analysis
    • Physical condition commensurate with the demands of the position

    Requirements

    • A master’s degree in Social Work from a graduate school approved by the New York State Department of Education and must be licensed as a Licensed Master Social Worker (LMSW) at time of interview
    • Successful clearance of criminal background check facilitated through New York State Office of Mental Health
    • For DCYF location, successful clearance from the State Central Registry in the New York State Office of Children and Family Services (Background investigation is conducted by the appointing authority)
    • Possession of a valid New York State driver’s license
    • Must be licensed as a Licensed Clinical Social Worker (LCSW) within four (4) years of interview
