Shaker Place Rehabilitation and Nursing Center’s admissions process focuses on learning as much about you or your loved one as possible. Our goal is to provide quality care the entire time you’re with us, starting with the admissions process which begins with a simple application, or give us a call at (518) 869-2231. We take the time to walk you and your loved one through it step by step, answering questions or offering assistance as needed.
To start, prospective residents should contact the admissions office for a basic clinical review of diagnoses, current physical functioning and care requirements. This is a necessary step allowing Shaker Place to ensure we can provide the highest quality of care to meet your individualized needs.
We are now offering an online admissions process where we will virtually walk you through the application, step by step.
All new residents will move into our fully renovated wings.
Shortly after your admission is complete, we’ll schedule a care/discharge plan meeting. It’s our way of coordinating your care during your stay, whether it be short or long-term. You can expect this meeting to occur within 7 to 10 days of your admission. It’s a great opportunity for you to meet our expert, caring staff from:
