Resident Care Coordinator

Salary: $83,718/annual

Schedule: Full-time - Full-time/dayshift (Monday – Friday)

Location: 100 Heritage Lane. Albany, NY 12211

The employee in this position, a registered nurse, is responsible for ensuring completion of minimum data set (MOS) assessments and related documents including care area assessments (CAAs) and comprehensive care plans. The work is performed under the general direction of the executive director. Does related work as required.

Responsibilities/Typical Work Activities:

  • Coordinates all (MOS) assessments for the nursing home by determining schedules;
  • Reviews and validates all MOS assessments;
  • Maintains records of MOS scores as a check against qualifiers;
  • Reviews all residents’ MOS assessments, ensuring completeness and accuracy;
  • Reviews eligibility for Medicare, determines Medicare/HMO coverage status and completes necessary notification forms;
  • Assists registered nurses in the implementation and completion of care plans as needed;
  • Maintains records of all completed assessments;
  • Develops in-service training material on MOS, CAAs, care planning and related rules and regulations and conducts group and individual training sessions;
  • Schedules and conducts interdisciplinary team care conferences for each resident;

Full Performance, Knowledge, Skills, Abilities and Personal Characteristics:

  • Good knowledge of the rules, regulations and policies covering Medicare eligibility requirements;
  • Good knowledge of professional nursing techniques and their relation to medical practices and skill in their application;
  • Good knowledge of resident needs in a long-term care facility;
  • Good knowledge of facility operations and the need to coordinate all services for the well-being of the residents;
  • Ability to secure the cooperation of others;
  • Ability to understand and carry out technical, oral and written instructions.

Minimum Qualifications (EITHER):

Graduation from an accredited college or university with at least a Bachelor’s degree in Nursing, and two (2) years of clinical experience in professional nursing activities in a skilled nursing facility.

            OR

Graduation from an approved three (3) year school of professional nursing and three (3) years of progressively responsible clinical experience in professional nursing activities in a skilled nursing facility.

Graduation from an accredited college or university with an associate degree in nursing and four (4) years of progressively responsible clinical experience in professional nursing activities, three (3) years of which shall have been in a skilled nursing facility.
